Job Summary: This new position will be responsible to effectively identify, evaluate and address disease prevention and health promotion issues of our patient population while administering quality patient care. Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues. Perform stress testing; prepping patients for stress and starting and management of IV Dobutamine infusions. Educate patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications. Maintain accurate, and confidential medical records. Document all medical evaluations, diagnoses, procedures, treatment(s), outcomes, education, referrals, and consultations consistent with ACC and state regulatory standards. Facilitate evaluation of records by physician(s), peers, and Quality and Standards according to protocols, and receive and implement constructive directives
Participate in the care of inpatient and outpatient clients in an independent and interdependent role including rounding on and managing specific disease processes for inpatients; providing patient education; providing post procedural follow-up care. Implement a specialty service within the practice (i.e. congestive heart failure clinic, lipid clinic, anticoagulation clinic, pacemaker clinic, etc.) and create / implement a cardiac rehabilitation program.
Provide leadership in collaboration with the healthcare team to define and integrate best practice models for designated patient populations throughout the continuum of care. Serve as an expert in the areas of clinical practice, education and research. Improve patient outcomes by identifying processes that promote high quality, cost effective patient care. Perform related responsibilities as required.
Qualified candidates will have valid California RN and NP License with completion of accredited nurse practitioner graduate degree program with minimum 1-2 years cardiology experience. Excellent patient care skills, strong leadership and time management skills. Current BCLS card. EMR (EPIC) experience preferred.
