Heart Transplant Coordinator
The Transplant Coordinator identifies appropriate candidates upon referral and monitors requirements for candidate evaluation. Coordinate with multidisciplinary team regarding evaluation, peri-operative and post-transplant process for the candidate/patient. Evaluate medical, psychosocial and financial data for review by the transplant team and patient selection committee. Will assess the caregiver/family and manage educational deficits –instituting a learning program to meet family and patient needs. Work with candidate, referring physicians, other healthcare providers, payors and transplant team members. Comply with organ procurement networks and organizations’ policies and listing requirements. Provide data for listing statistics. Oversee program to ensure availability of accurate stats.
Interested candidates must have graduated from an approved school of nursing with a Master’s degree in Nursing or a health care related field. Requires 4 years of nursing experience with 2 of those years in critical care nursing with a minimum of 1 year of previous transplant experience. Must have a current RN license in the state of Florida and maintain CPR certification. A Transplant Specialty certification within 2 years from the date of hire is required.
