Cardiovascular Screener/Outreach Coordinator

Byrne Healthcare’s heart-disease prevention program serving the Phoenix community with patient consultation, education, and life-saving risk identification is accepting applications for the position of Cardiovascular Screener/Outreach Coordinator to work on a contract basis on-site at a Chandler hospital.
The ideal candidate for this important role must be passionate about all matters pertaining to preventative medicine and proactively involved in learning news ways to take action against the world’s largest killer – heart disease. The ideal candidate will possess an outgoing, positive personality with outstanding written and verbal communication skills and the desire to function in a variety of alternate settings such as community events, business functions, health fairs (mobile operations facility) and within the hospital are required. A well-organized and professional individual with the personality and drive to promote this initiative via referrals, speaking engagements, and their genuine enthusiasm for the program is imperative.
Responsibilities of this position include but are not limited to:
- Pre-qualifying individuals for the HeartAware program
- Scheduling individuals for clinical consultation, including any follow-up appointments as necessary
- Screening individuals for heart disease risk factors by building a profile via a secure web-based application, sampling blood, creating a plan of action, and triaging a referral if necessary
- Performing basic computer functions (Excel, Internet)
- Communicating activities, experiences, recommendations and suggestions to the marketing department
- Referring patients to appropriate channels (PCP, Cardiologist, wellness classes, other hospital programs) after the consultation is complete
- Complying with all policies and procedures regarding patient care, incident reporting, quality control, and applicable local/state/federal regulations
Education, Experience and Skills:
ADN, Diploma, BSN or foreign language equivalent required
Previous experience with patient care modules or PC required
Knowledge of data analysis techniques
Two years experience with coronary care and/or cardiac telemetry experience preferred
Licensure/Certification:
Current AZ State RN License in good standing
Current BLS card
Current ACLS card
